Qualifications

• Degree or Diploma in social work, counselling, psychology or related fields required.
• Preferably 2-3 years of experience in healthcare or community care.
• Prior experience in casework with persons living with dementia is an advantage.

Responsibilities

• Provide basic case management for persons with dementia and their carers.
• Coordinate care and resources for dementia clients.
• Plan and conduct outreach and case finding for dementia.
