St. Hilda's Community Services Centre is a voluntary welfare organisation. We are a Senior Care Centre with a Day Rehabilitative Centre and an independent Drop-In Centre serving the needs of the community in one of Singapore's early housing estate in the locality of Tanjung Rhu and its surrounds.

Job Summary

You will deliver nursing care and management to clients enrolled in both centre-based and home-based services, leading junior nurses and nursing students to achieve optimal patient outcomes across diverse healthcare settings.

Responsibilities
  • Deliver comprehensive nursing care to clients in centre-based and home-based settings to support positive health outcomes
  • Lead and supervise junior nurses and nursing students to ensure adherence to care standards and enhance patient experiences
  • Apply nursing expertise across acute care, primary care, integrated care, and long-term care facilities to meet client needs
  • Collaborate effectively with interdisciplinary healthcare teams to coordinate care plans and improve service delivery
  • Demonstrate resourcefulness and systematic problem-solving to address client care challenges and improve operational efficiency
  • Foster a motivated and cooperative team environment by supporting and engaging with team members and healthcare professionals
  • Provide administrative and operations support
