Job summary

A healthcare provider in Singapore is seeking a Diploma holder to coordinate patient care with a focus on ERAS (Enhanced Recovery After Surgery) implementation. The role requires good communication skills, experience in nursing or clinical areas being a plus. Responsibilities include tracking patient outcomes and facilitating communication across healthcare teams. This position will be deployed to one of three hospitals.

Qualifications

  • Prior working experience in Nursing/Clinical or patient care areas will be an added advantage.
  • Able to work meticulously and independently.

  • Coordinate pre-operative assessments and patient education sessions and materials.
  • Provide pre-operative counselling and education to patients and families.
  • Monitor post-operative recovery and coordinate follow-up care.

Responsibilities
  • ERAS (Enhanced Recovery After Surgery) Implementation and Care Coordination (45%)
  • Implement and monitor ERAS protocols across designated surgical conditions
  • Coordinate pre-operative assessments and patient education sessions and materials
  • Provide pre-operative counselling and education to patients and families
  • Provide post-operative discharge support services to patients within 30 days of discharge
  • Monitor protocol compliance and document variances
  • Coordinate care across multidisciplinary teams throughout the patient journey
  • Track and follow up on patient outcomes and recovery metrics
  • Facilitate communication amongst various healthcare providers
  • Patient Care Management (25%)
  • Conduct pre-operative screening and baseline assessments
  • Provide patient education and counselling throughout the care journey
  • Coordinate pre-habilitation programmes job description of the VBC CPEs when required
  • Monitor post-operative recovery and coordinate follow-up care
  • Facilitate discharge planning and transition of care
  • Provide post-discharge support and follow-up (e.g. Home rehab and Home nursing services)
  • Data Management and Quality Improvement (15%)
  • Collect and maintain data for ERAS compliance and outcomes
  • Track key performance indicators including length of stay and readmission rates
  • Support the implementation of Patient Reported Outcome Measures (PROMs)
  • Participate in quality improvement initiatives
  • Generate regular reports on ERAS performance metrics
  • Assist in analysis of outcomes data
  • Participate in departmental meetings and quality improvement projects
  • Support the implementation of Value-Based Care initiatives
  • Participate in staff training and education sessions
  • Support any other Value-Based Care related work as required
  • This position will be deployed to one of the three NHG hospitals (Khoo Teck Puat Hospital, Tan Tock Seng Hospital, Woodlands Health).
