Job summary

A leading healthcare provider in Singapore is seeking a dedicated nurse to provide patient education and manage chronic diseases. Responsibilities include counselling on health promotion, nutritional assessments, and conducting screenings. The ideal candidate has a diploma/degree in nursing, is registered with the Singapore Nursing Board, and possesses excellent interpersonal skills. You will work closely with care coordinators and potentially assist doctors in various clinical activities. Qualifications

• At least 5 years of clinical nursing experience.

Responsibilities

• Provide nurse counselling and patient education on chronic disease management.
• Assess patient’s nutritional needs and provide nutrition education.
• Conduct Diabetic Foot Screening and Foot Care education.
