Job description

Case Management Officer responsible for Project Renalssance which aims to demonstrate that early introduction of palliative care options for this irreversible disease will be effective, scalable, economically sustainable and have a significant impact on the quality of lives of advanced CKD patients.

Responsibilities
  1. Assist with overall coordination of appointments to Kidney Supportive Care Clinic and referral to community partners (Home hospices/ integrated home health etc.)
  2. Deliver patient care (inclusive of education and counseling) according to the established standards of care and practice using patient care plan / clinical pathways.
  3. Contribute to multidisciplinary patient management.
  4. Apply understanding of prevention of medico-legal issues in relation to patient care.
  5. Collaborate and co-operate with physicians and other health-care providers in the delivery of patient care and services.
  6. Demonstrate proper use and care of equipment.
  7. Demonstrate proper and economical usage of consumables and supplies.
Qualifications
  1. Degree in Nursing, Biomedical Science, Life Sciences, Biotechnology or General Science.
  2. A certificate / diploma in Renal Nursing will be advantageous.
  3. Registered Nurse with at least 3 years of clinical experience.
  4. Able to demonstrate clinical skills, knowledge and critical thinking in delivery of patients’ care.
  5. Possess good interpersonal and communication skill.
  6. Computer literacy skills – MS Office (Word / Excel / PowerPoint).
  7. Effectively bilingual.
