Job description
Job purpose

The jobholder is primarily responsible for supporting the seamless delivery of integrated care for seniors by conducting standardized InterRAI assessments and coordinating with Care Planners, Care Coordinators, and community partners (SCCs, AACs, HPC+, HT) to ensure holistic support.

Duties and responsibilities
  • Make first contact with clients (e.g., via home visits) to conduct initial assessments.
  • Administer InterRAI Check-ups for clients in accordance with InterRAI standards, supplemented with accurate data from clients, caregivers, and available documentation (e.g., health records, social reports, electronic systems, and case notes) where relevant.
  • Conduct re-assessments for clients on a 6-monthly basis or as needed.
  • Ensure compliance with documentation standards for InterRAI assessments.
Qualifications Required
  • Bachelor’s degree in healthcare management, gerontology, or a related field.
  • Healthcare or Allied Health professionals (e.g., nurse, social worker, therapist) OR
  • Care staff with prior experience (> 2 years) in assessments, care coordination and/or case management, preferably in senior or long-term care sector.
  • Have experience using InterRAI system.
