We are looking for a Social Worker who delivers patient-centred psychosocial support, contributes to social care quality across the clinic and works as an integrated member of the multidisciplinary team.

The Role

The Social Worker is responsible for planning, coordinating, delivering and evaluating social care services for patients across one or more assigned clinics. The role includes psychosocial support, patient well-being programmes, health promotion and educational activities, community engagement, social research and preventive care services, in line with Diaverum clinical governance and applicable local and international standards.

You will report directly to the Clinic Medical Director for clinic objectives and functionally to the Head of Social Care for standards of care and quality management. You will work closely with the multidisciplinary team, clinic nurses, physicians and operations management.

What You Will Do

You will provide direct patient support and contribute to quality, documentation, education and continuous improvement across your assigned clinic or clinics.

Your responsibilities will include:

  • Provide psychosocial support, health promotion and lifestyle management tailored to hemodialysis patients with different demographic backgrounds, medical histories and psychosocial needs. Work with patients and the multidisciplinary team to support treatment adherence, including attendance, treatment duration, medication and required lifestyle changes.
  • Implement a continuous, accurate and effective Social Care Process for all patients, including screening, assessment, diagnosis, intervention and counselling, documentation, monitoring and evaluation.
  • Coordinate patient support programmes that improve access to social services, financial aid, transportation assistance and psychosocial interventions, and participate in relevant community engagement activities in alignment with the Head of Social Care.
  • Contribute to patient experience and quality-of-life initiatives by collecting and reviewing patient feedback, identifying areas for improvement and supporting actions that improve the overall care experience.
  • Maintain accurate and complete documentation, including Monthly Care Plans, social care follow-up records and in-clinic referrals. Monitor relevant psychosocial indicators and contribute to evaluating social care outcomes.
  • Comply with policies and procedures relating to scope of care, patient confidentiality and rights, infection control, facility management and safety, as well as JCI, CBAHI and applicable local regulatory requirements. Monitor compliance with social care policies and procedures.
  • Work closely with the Head of Social Care, Clinic Medical Director, clinic team and operations management to support quality goals and social care parameters. Participate in social care department meetings, clinic management meetings, quarterly management reviews, monthly business reviews and clinic development initiatives.
  • Participate in social care research, quality improvement initiatives, training and continuous professional development. Support the orientation and induction of new social work team members, as applicable.

Experience and Qualifications

We are looking for someone with a social work background and a genuine interest in clinical and patient-centred care.

What we need:
  • Bachelor's degree in Social Work. A master's degree in Social Work is favourable.
  • Certified Clinical Social Worker Specialist by the Saudi Commission for Health Specialties (SCFHS).
  • Fluent in Arabic and English, spoken and written.
  • Proficient in MS Office and basic computer systems.
Experience and capabilities:
  • Fresh graduates are welcome. One to two years of social work experience in a clinical setting or hospital is favourable.
  • Ability to work within a multidisciplinary healthcare team.
  • Ability to document and manage patient care plans, psychosocial assessments, follow-up records and referrals.
  • Understanding of JCI, CBAHI and applicable local quality and compliance requirements.
What gives you an edge:
  • Experience in renal care, diabetes, hypertension or chronic disease management.
  • Familiarity with psychosocial indicators and social care measurement and evaluation.
  • Experience in patient experience and quality-of-life initiatives, including patient feedback and service improvement.
  • Experience with patient support programs covering financial aid or transportation services.
  • Prior involvement in social care research or quality improvement projects.
