A home care service provider is seeking two experienced Case Coordinators to manage Home Care Packages in Perth. This role involves liaising with hospitals, conducting assessments, and ensuring compliance with Aged Care legislation. Ideal candidates will have prior experience in home care coordination and strong interpersonal skills. The position offers above award pay rates and a supportive team environment.

We are seeking two (2) experiencedCase Coordinator to join our Perth team, responsible for supporting clients in Perth, Toodyay and the surrounding areas. This role is ideal for someone who has experience coordinating Government-funded Home Care Packages (HCP) and who is passionate about delivering client-focused care that supports independence, dignity, and wellbeing.
As a Case Coordinator, you will work primarily from our Perth office, with work conducted mainly over the phone. Some travel to Toodyay and the Wheatbelt will be required, and a business car will be available when needed.
Your role will involve liaising with hospitals for new referrals, sourcing external providers, and supporting the growth of our services through professional networking.
