Job summary

A regional health service provider is looking for a Community Mental Health Team professional in Macclesfield. The ideal candidate will have a diploma/degree and experience in mental health care. Responsibilities

  • Develop relationships with service users and partners.
  • Promote areas for development within the CMHT.

Job description

A Community Mental Health Team (CMHT) professional will primarily undertake a duty function that includes completing new assessments and responding to immediate needs of individuals already open to the team.

Responsibilities
  • Triage and assess referrals.
  • Collaboratively identify and manage risk.
  • Provide timely and appropriate support/interventions to service users experiencing crisis.
  • Develop positive working relationships with service users, carers, GPs, third sector partners and the multidisciplinary team.
  • Ensure clear channels of communication with clients, carers, GPs, Social Services and other partners.
  • Promote and identify areas requiring development within the CMHT and take a lead role in the development of services in conjunction with the Team Operations Manager, Head of Clinical Services and other professionals.

All new starters will commence in post on one fixed start date each month in line with our Prepare to Care (P2C) induction programme. The induction offers up to 3 weeks training to support meeting initial training requirements of the role.

Qualifications

Essential criteria:

  • Educated to diploma/degree level and registered with the appropriate professional body (e.g., Registered Mental Health Nurse, Occupational Therapist).
  • Professional knowledge acquired through a relevant degree plus relevant experience for working with complex cases.

Desirable criteria:

  • Evidence of continued professional development.
  • NVQ A1 qualification.
  • Relevant mentorship qualification as required by governing body.
  • Specialist practitioner qualification (SPQ) and in‑depth knowledge and experience of working within the recovery‑focused philosophy framework.
  • Knowledge of delivering individual service‑user focused specialist assessments.
  • Knowledge of evidence‑based interventions measurable and promoting the service user’s journey along the recovery pathway.
  • Knowledge of delivering psychosocial interventions (PSI).
  • Understanding of physical health needs.
  • Experienced in delivering specialist programmes of care, care packages.
  • Knowledge of providing specialised advice in relation to care.
  • Knowledge of recording personally generated information.
  • Ability to work closely with commissioners and care providers.
  • Knowledge of legal frameworks including the Mental Health Act and Mental Capacity Act.
  • Experience of undertaking surveys, audits and research and development activity.
  • Minimum of 2 years post‑registration experience working with adults with mental health problems of a complex nature.
  • Experience of planning and organising complex activities or programmes requiring formulation and adjustment.
  • Experience of developing specialist programmes of care, care packages.
  • Experience of safe management of equipment and resources.
  • Experience being an authorised signatory.
  • Experience of conducting professional/clinical supervision.
  • Experience of undertaking equipment testing and adaptation.
