Job description
Role Overview

We are looking to appoint a skilled and motivated Mental Health Practitioner to work in one of the four Primary Care Network locations across Greenwich. This role is appointed under the Additional Roles scheme and works closely with general practice staff. The service is rapidly developing and each practitioner carries a named role for GP practices in the area.

Key Responsibilities
  • Work with community partners to improve the lives of people with mental ill health by delivering an excellent, compassionate service promoting recovery and well‑being of patients and their carers.
  • Support the delivery of PCN DES contract priorities, including improvement of physical health for people with serious mental illness.
  • Facilitate the interface between primary care and secondary mental health services, providing specialist advice, assessment, treatment, education and solution‑focused approaches to local primary care teams.
  • Offer networking and navigation expertise to promote access to specialist mental health support services.
  • Accept referrals from primary care via an agreed route, promote early assessment and treatment, and maintain robust relationships with community‑based services.
  • Provide dedicated support to primary care, giving timely advice to GP practice staff.
  • Work with adults (18+) whose mental health needs are best met within primary care, using a biopsychosocial model.
  • Follow up on patients with serious mental illness.

Oxleas is an NHS provider delivering community health care within Greenwich, London, and surrounding areas. The practitioner will work across GP practices and Oxleas sites in Greenwich.
