A community health service provider is seeking a Band 6 Senior Community Nurse to join a new Hospital at Home service in Worcestershire. This role requires strong independent working skills and the ability to manage acutely unwell patients in the community. The successful candidate will collaborate with healthcare teams and stakeholders, ensuring efficient patient care delivery. We have an exciting opportunity for Band 6 Senior Community Nurses who have a passion for admission prevention to join a core Team of ACPs to deliver a new and developing countywide Hospital at Home (H@H) service in Worcestershire. These are full time posts, but we would consider applicants looking for part time. Posts are based in the Single Point of Access (SPA) Worcester. This is an exciting opportunity to be part of the delivery of a new service and to play a significant part in shaping and developing this service. There will be the opportunity to partake in audit and service development as well as the clinical duties of the post. There will also be the opportunity to work collaboratively with stakeholders in particular with Acute Trust colleagues. The service will be countywide across Worcestershire so the ability travel across the county will be expected. The service will be operational 8am-8pm seven days a week including Bank Holidays so the post holder will be expected to work shift patterns to enable cover of the operational hours.
Main duties of the job: There will be the requirement to support the assessment and management of acutely unwell patients in the Community who otherwise might have been in an Acute Hospital setting with the support of a dedicated medical model for the Hospital @ Home. The ability to work independently is essential as the post holders will need to offer urgent care, treat minor illness, give patient advice and offer chronic disease management. You will have the ability to prioritise and triage the needs of patients, making any referrals for further investigations where appropriate in line with local and practice guidelines, whilst ensuring your clinical practice is safe and effective and escalating to clinical leadership as required.
