Social Worker (Adult Social Care Team)

Location: Wokingham
Rate: £35.89 per hour
Team: Adult Social Care (18+)

Wokingham Borough Council is seeking an experienced Social Worker to join the Adult Social Care Team. The role involves working directly with adults aged 18+, including those with learning disabilities, physical disabilities, long-term conditions, dementia, drug and alcohol-related needs, homelessness, hoarding behaviours, and individuals transitioning into adulthood or older age. You will also support young carers and adult carers, ensuring they can continue their caring role safely and sustainably. The core purpose of the service is to enable people to live safely in the community, maximise their strengths and abilities, and promote independence wherever possible.

Key Responsibilities
  • Complete Care Act assessments, reviews, and support plans
  • Undertake safeguarding enquiries (S42)
  • Work with individuals with complex needs and their carers
  • Engage in multidisciplinary working across health and community services
  • Support people to achieve personal outcomes and maintain independence
  • Carry out office-based work at least once per week and visits on other days
Candidate Requirements
  • Qualified Social Worker with a minimum of 3 years' experience in Adult Social Care
  • Experience in dementia, drug/alcohol issues, homelessness, hoarding, safeguarding, carers assessments, community work, and MDT working
  • SWE registration
  • Car driver with access to a vehicle
  • Ability to work both independently and collaboratively
