Overview

The Role
We are seeking dedicated Support Workers to join our West Lothian Outreach service. This is a person-centred position, supporting adults with learning disabilities to live independently, achieve meaningful outcomes, and participate in their local community.

Responsibilities
  • Providing care and support, including personal care, medication administration, shopping, housework and cooking.
  • Assisting individuals to maintain and develop independent living skills.
  • Offering social support to promote community involvement and inclusion.
  • Supporting the development and maintenance of positive relationships.
  • Liaising with medical professionals when required.

An enhanced Adult PVG is essential for this role.

Shift Patterns
  • Day Shifts: 8am–3pm, 9am–3pm, 10am–3pm, 11am–3pm
  • Afternoon/Evening Shifts: 3pm–9:30pm
  • Waking Nights: 9:30pm–8:00am

Additional Information

Offering care and support to one adult with learning Disabilities to ensure the running of the home, health and wellbeing. This will include limited Personal Care, medication, house work, cooking and most importantly maintaining and developing his independent skills. Social support encouraged daily to ensure involvement and independence within his community and shopping tasks as well as offering new experiences. Support plans/ Risk Assessments in place to ensure continued accurate support. You will be required to liaise with medical professionals when required.
