Make a difference to those who require Urgent Community Response in their own homes - with enthusiasm, energy and passion - we are looking for Band 6 Registered Nurses, Paramedics or Therapists who have experience in community or tertiary services and want to continue developing as independent practitioners. (Newly Qualified staff cannot be considered for this role).
Please note interviews will be face to face only. There will be 2 interview dates in January (6th and 20th January) to facilitate the recruitment of these posts

Community experience is not essential but applicants will need to demonstrate the Trust core Values for CNWL and West London Trust and that they understand the issues involved in working in people's own homes, working alone and being able to work outside of regular working hours, weekends, public holidays and face to face visits will be essential.
If successful you will be providing holistic care to service users in their own homes and across our pathways of Urgent Community Response/Hospital at Home and Discharge to Assess. You will be required to be flexible, be team and patient focused and be able to work independently as needed. The Therapy staff who are employed will be working with primarily within the Discharge to Assess pathway - ensuring patients are safe in their home following an acute admission to hospital, but providing therapy input across the other pathways.
This position will require you to visit various locations throughout the London Boroughs of H&F, K&C and Westminster. We run a seven-day service and our working hours are 8 am until 10 pm/365 per year, which you will need to be able to fulfil required shift patterns including weekend working and out of hours working.
