Job description

The Triage Nurse role is integral to ensuring that all patient requests into the practice are clinically assessed and prioritised accordingly. The post holder will provide self-care advice appropriate to the patients needs or allocate them to the most appropriate practitioner or service.

Main duties of the job

Working as part of the Triage team, the post holder will provide clinical triage of all e-consultations, telephone calls and home visit requests where needed.

The post holder will identify the clinical need and urgency to provide a time frame for intervention, allocating them to the most appropriate clinician or service to meet the needs and will provide advice and signposting as required.

There may be, on occasion, a requirement to carry out other tasks.

This allows us to assess all requests promptly, direct patients to the right clinician or service, and prioritise urgent needs effectively. By using this system, we aim to offer safe, efficient, and timely care while making the best use of our teams expertise.

Job responsibilities

The post holder will utilise clinical triage and assessment skills, whilst working within the Scope of Professional Practice and adhering to local policies/guidelines. The post holder will work as part of a multidisciplinary team designated to respond to patients via online consultation, 111 referrals or phone calls to decide on best plan of care, this could include advice over the phone, signposting to other services or offering appointments.
