A healthcare organization providing mental health services is looking for a Transitions Nurse in Southampton. This role requires a degree-level qualification along with current Nursing and Midwifery registration. The successful candidate will support young people during their inpatient admission and assist with their transition back to the community. As a Transitions Practitioner, you will play a key role in supporting young people and their families/carers throughout their inpatient admission process and assist with discharge planning, offering continued support when they return to the community. This is an exciting opportunity to work within a unique and specialised CAMHS team that provides support both in the hospital and the community.
We adopt a holistic approach, not only focusing on mental health but also addressing the physical, social, educational, and motivational needs of the young people we work with. By employing the "social prescribing" model, we help young people engage with life through activities that promote community connections, sports, education, employment, housing, and more. Your role will empower young people to build confidence, improve wellbeing, and work towards their long‑term health goals.
Assist and support the service and Multi‑Disciplinary Teams by participating and leading in the treatment and support of young people who have been inpatients in Serenity Path Hospital.
To lead in supporting young people who are being transitioned between inpatient CAMHS and adult mental health services, in order to enable a seamless and person‑centred transfer of care.
Working within the inpatient and community teams to develop plans of support appropriate to individual young people.
To lead the young person’s integration from hospital to home, and back into outdoor activities/clubs, hobbies and educational provisions using the model of social prescribing. The postholder would be required to participate in the activities along with the young person.
To facilitate parental and professional highly specialist training on self‑harm (FLASH) and suicide prevention as previously offered to schools, hospital and social care professionals.
To work alongside qualified practitioners in the delivery of ongoing therapeutic group work within the young person’s transitional period between inpatient and community settings in order to maintain continuity of care.
To work closely with the multi‑agency partners including education, YOT, third sector services, community CAMHS teams, social care and etc.
To maintain your own diary and appointments, managing your own caseload and maintaining communication with the whole network involved.
