Job description
Overview

We are looking for someone to join the team to coordinate individuals' transition from School to College, College to College or within WCG to support and facilitate students with learning difficulties and/or disabilities on their learning journey. This post will sit within the Supported Learning Department and will report directly to the Head of School. May include but are not limited to:

Responsibilities
  • Develop and deliver a bespoke transition for high needs students. This transition starts from when the student has an interest in the college and finishes when the student is settled on the correct course. If necessary, help with transition to a more appropriate course.
  • Determine transition needs by the Education, Health and Care Plan and associated costs.
  • Conduct regular transition visits.
  • Maintain contact with the current education setting and parents/carers; maintain regular contact with local authorities.
  • Explore different courses and options for students.
  • Create partnerships with local authorities and local specialist provisions.
  • Keep regular transition notes and input information onto shared spreadsheets.
  • Develop working partnerships with SEN specialist provisions and mainstream settings, so they are aware of the college offerings and available support.
  • Visit local SEN schools to meet and build relationships with Inclusion staff.
  • Be based in Rugby but travel to all Warwickshire College sites and to potential feeder schools.
  • Attend and participate in Education, Health and Care Plan reviews for prospective and current students.
  • Maintain regular contact and meetings with the Head of School to ensure the correct support is in place and costs are correct.
  • Attend college Open Events across all sites as necessary.
  • Arrange group visits during the summer term for students on similar courses to meet their teachers and support staff ahead of September.
  • Record all information, dates and contact details on Transition files and Inclusion spreadsheets; ensure admin is recorded and shared with necessary people to keep Inclusion Leads up to date with information on new students.
  • In order to support during busy exam periods, you may be asked to undertake exam invigilator duties. The College will provide training.
  • The list of duties is not exhaustive and may evolve without changing the nature of the duties or the responsibility level.
Qualifications
  • DBS check (Enhanced) required; prior to appointment, an Enhanced DBS check is needed as you will be working within Regulated Activity.
  • Excellent communication skills and ability to work with a range of outside organisations; travel to Schools and Colleges within the area may be required.
  • GCSE Grade C or above in English and Mathematics, or Level 2 equivalent qualification; a professional, academic or vocational qualification at Level 2.
  • Working knowledge of information technology (e.g., email, internet, and other software applications).
  • Excellent administrative and organisational skills.
  • A driving licence free of major endorsements and the use of a motor vehicle.
  • Team Teach Trained or willingness to gain the qualification.
  • Experience of working with SEND students.
  • Knowledge of SEN Code of Conduct and the Education and Health Care Plan.
Desirable Attainments
  • Experience of working in a Further Education setting.
