Job description

To support the practice nursing team in the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The healthcare assistant will deliver and assist clinical staff in the provision of treatment, preventative care, health promotion and patient education

Main duties of the job

This role plays a vital part in supporting high-quality patient care and the day-to-day running of the practice, combining hands-on clinical support with essential administrative and clinical systems duties.

  • Supports the delivery of patient care through a mix of clinical and administrative responsibilities.
  • Carries out patient-facing tasks such as new patient health checks, baseline observations, ECGs, wound care, specimen handling, chaperoning, and support during clinics and minor procedures.
  • Provides support to nurses with health promotion activities, chronic disease management, care plans, and opportunistic patient education.
  • Undertakes additional clinical tasks following training, including venepuncture, vaccinations, ear irrigation, and blood pressure monitoring.
  • Ensures clinical rooms are prepared, stocked, and maintained in line with infection prevention and control requirements.
  • Maintains accurate and up-to-date patient records by summarising, coding, scanning, and filing clinical correspondence.
  • Manages call and recall systems, appointment templates, and clinic setup to support efficient patient flow.
  • Runs audits, clinical searches, and data quality checks to support reporting and service improvement.
  • Supports the effective use of the practice clinical system, including user management, troubleshooting issues, system updates, backups, and data security.

Job responsibilities

Primary Responsibilities

The following are the core responsibilities of the healthcare assistant. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

  • Undertake new patient health checks
  • Support the practice nurse with health promotion programmes
  • Carry out baseline observations such as pulse oximeter, blood pressure, temperature, pulse rate, recording findings accurately
  • Facilitate routine and 24-hour BP monitoring, advising patients accordingly
  • Undertake wound care, dressings and other clinical tasks as required
  • Support the practice nurse with the management of chronic disease clinics
  • Carry out BMI checks as directed
  • Act as a chaperone as required
  • When trained, undertake venepuncture
  • When trained, administer vaccinations as appropriate
  • When trained, carry out ear irrigation under the supervision of the practice nurse
  • Carry out ECGs as requested
  • Ensure specimens are recorded and ready for onward transportation
  • Provide support during minor operations as required
  • Ensure all clinical rooms are adequately stocked and prepared for each session
  • Ensure fridges are cleaned routinely in accordance with extant guidance
  • Ensure clinical waste is removed from clinical areas and sharps bins replaced in accordance with the practice IPC policy
  • Deliver opportunistic health promotion where appropriate
  • Complete Care Plan

NON CLINICAL

  • Reviewing medical records and producing an accurate summary of the patients medical history.
  • Read Coding the information into the practice clinical system
  • Auditing data collection standards in the practice
  • Monitoring progress of notes summarising against practice targets
  • Monitoring patient call and recall systems
  • Filing and retrieving paperwork
  • Extraction of necessary data from clinical correspondence and input into electronic patient Record.
  • File copies of hospital referrals.
  • File hospital letters.
  • Scanning of letters and relevant correspondence into patients notes.
  • Summarisation of patients notes when requested.
  • To ensure the efficient running of the practice computer system, carrying out searches, audits and recalls as required by the doctors, nurses and manager.
  • Troubleshoot clinical database
  • Load updates
  • Report faults to software supplier and track progress
  • Ensure backups are taken in accordance with practice procedure
  • Pro-actively initiate data quality improvement projects
  • Set up and run clinical searches
  • Ensure accurate report output
  • Interrogate data for audit purposes
  • Maintain the appointments system
  • Set up clinic templates
  • Support and guide members of staff in the use of the computer system
  • Ensure security of data at all times

Secondary Responsibilities

  • Participate in practice audit as directed by the audit lead
  • Participate in local initiatives to enhance service delivery and patient care
  • Support and participate in shared learning within the practice
