Package Description:

Shift Details - Night shift with alternate weekends

As a Team Leader at Avery Healthcare, you will support the home management team in delivering high-quality, person-centered care. You will help create a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff. Key responsibilities include leading the person-centred care planning process, assisting with resident discharges, monitoring resident well-being, and ensuring safe systems of work.

Other responsibilities include:

  • Leading in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
  • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping residents\' next of kin fully informed, where appropriate.
  • Monitoring the well-being of each resident, being aware of any changes impacting care and care planning, and ensuring that changing needs are met and written records reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
