A local government authority in the UK is seeking a Family Support Practitioner to provide effective support to children and families through varied family support services. This role involves assessing needs, coordinating care, and collaborating with multi-agency partners. The ideal candidate will show commitment to diversity and possess strong communication skills. To provide support to children, young people and their families through the delivery of a varied family support offer in order to improve outcomes for children aligned to the Supporting Families Programme. Your role will be to undertake Early Help assessments and act as Lead Practitioner, to assess, co‑ordinate, plan and deliver integrated and intense family support as part of a multi‑agency early help plan. You will be based in one of our Family Hubs.

In this role, you will work closely with partners across health, education, and the voluntary sector to deliver a whole‑family approach, supporting positive outcomes for children and young people.
