About The Role

As a PRT Support Worker, you’ll play a vital part in supporting vulnerable adults in the community, helping to prevent homelessness and increase independence. You’ll manage a caseload of clients, delivering high-quality, person-centred support to help them achieve positive outcomes.

Your Work Will Include
  • Delivering tailored support to clients with a wide range of support needs which may include mental health, learning difficulties, substance use, and complex needs.
  • Creating, implementing, and updating Support Plans to achieve timely and effective outcomes.
  • Providing practical housing-related assistance, such as helping clients maximise income, apply for welfare benefits, prepare for or find work, manage household tasks, reduce isolation, and access other services.
  • Working creatively and flexibly to engage clients, identify unmet needs, and signpost or escalate as appropriate.
  • Communicating sensitively and effectively, supporting clients to manage emotions while maintaining a focus on practical outcomes.
  • Promoting independence, skills, and confidence in clients.
  • Maintaining accurate electronic records and contributing to case reviews and meetings.
  • Contributing to service improvement, performance targets, and the induction and training of colleagues.
About You
  • Experience supporting vulnerable people with a range of needs, working effectively on your own initiative.
  • Knowledge of housing, homelessness, and welfare benefits, with the ability to keep up to date and seek specialist advice when needed.
  • A positive, flexible approach and a commitment to personal development and learning.
  • The ability to assess support needs, write and implement Support Plans.
  • Good literacy and numeracy skills, with the ability to support clients with correspondence, finances, and budgeting.
  • Sensitivity to the emotional needs of clients, with a focus on practical outcomes and promoting independence.
  • Strong communication and relationship-building skills, able to work jointly with colleagues, carers, and external agencies.
  • Understanding of equalities and diversity issues.
  • Working knowledge of Microsoft 365 or equivalent, and a willingness to learn new technology.
  • Ability to converse with citizens and provide complex information in accurate spoken English, or through a BSL interpreter.
  • Willingness and ability to travel throughout the city to support people in their homes and in the community.
