Are you ready for a role where every day truly matters? Join us as a Rapid Response Support Worker and play a vital part in enabling clients with complex care needs to return home from hospital and live safely, comfortably, and independently with their families. This is more than a job – it's a chance to transform lives.

Why This Role Is So Rewarding

You’ll be at the heart of helping clients transition from hospital or other care settings back to the place they belong – their own home. Your support ensures they receive high-quality, compassionate care while regaining control of their lives. Few roles offer such a direct and meaningful impact.

As a Rapid Responder, you will:
  • Support the start-up and stabilisation of new complex care packages across the UK
  • Assist Care Managers and clinical teams with safe and efficient hospital discharges
  • Step in where needed to provide continuity, stability, and reassurance to clients and their families
  • Mentor new Support Workers, role-modeling best practice and safe, effective care
  • Deliver personal care and daily living support tailored to each client
  • Establish routines, organise essential supplies, and manage prescriptions or consumables when required
  • Maintain the highest level of professionalism and clear communication at all times
What Makes This Role Unique

Flexibility is key. You may:

  • Work live-in shifts for consistency, sometimes up to 3+ weeks at a time
  • Work live-out depending on client needs (accommodation may be provided depending on location)
  • Travel across the UK to help launch new care packages
  • Work occasional unsociable hours based on client requirements
What We're Looking For
  • Full UK Drivers Licence
  • Proven experience in care; complex care experience highly desirable
  • A self‑motivated, organised, and flexible approach
  • Strong communication skills, both written and verbal
  • The ability to work independently and as part of a team
  • Commitment to maintaining professional boundaries
  • Willingness to travel and stay in different locations as needed
  • Experience supporting clients with hospital discharges (desirable)
  • Level 3 or above in Health & Social Care qualification (desirable)

Your support can bring a family back together – don't miss the chance to make that happen.

If you have the passion and drive to give someone the gift of returning home safely, we're ready to hear from you right now!
