Key Responsibilities
  • Deliver Tailored Support: Provide responsive, person-centred assistance to individuals with complex needs, helping them achieve positive outcomes and independence.
  • Foster Collaboration: Build open and trusting relationships with customers, colleagues, and partner agencies to ensure seamless and comprehensive support.
  • Promote Participation: Encourage individuals to engage in social, educational and employment opportunities, preventing isolation and promoting self-determination.
  • Manage Risks Effectively: Conduct thorough risk assessments and maintain professional boundaries, ensuring health, safety and compliance with safeguarding policies.
Qualifications
  • Have an understanding of support needs and support planning.

This role is being offered on a part time, permanent basis working 22.2 hours per week.

You will be working on a rolling 5 week rota, Monday to Sunday between 8am and 9pm on a 4 week rolling rota.

This is a scheme based role working 3 days per week. This role will be based in the Truro/Falmouth/Penryn area.
