A healthcare organization in Bournemouth seeks a professional to join the Community Rehabilitation Team, supporting service users in their recovery and social inclusion. Responsibilities include implementing individualised My Wellbeing Plans and maintaining high standards in record-keeping while ensuring confidentiality. The Community Rehabilitation Team will support people to reach an optimum level of function, focusing on activities of daily living, vocation, and leisure. The post holder will work towards these goals and provide care for people both in the community and for those planning discharge from the Rehabilitation Inpatient Units with severe mental illness who have multiple and complex needs. This role is under the direction of qualified practitioners.
To provide support to an allocated group of service users, promoting their recovery and social inclusion and maintaining them in their community environment (STR role).

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service can present a valid DBS certificate instead of requiring a new check.
