Job description
Overview

The Community Rehabilitation Team will support people to reach an optimum level of function, focusing on activities of daily living, vocation, and leisure. The post holder will work towards these goals and provide care for people both in the community and for those planning discharge from the Rehabilitation Inpatient Units with severe mental illness who have multiple and complex needs. This role is under the direction of qualified practitioners.

To provide support to an allocated group of service users, promoting their recovery and social inclusion and maintaining them in their community environment (STR role).

Responsibilities
  • Focus on the direct needs of service users, working across boundaries of care, organisation, and role, coordinated through assessment and planning and implementing their individualised My Wellbeing Plan.
  • Offer intensive support for people requiring a recovery approach that is flexible according to need but without fostering dependence.
  • Work with trained staff to identify activities that the patient enjoys, finds meaningful and which occupy and divert them in a positive way.
  • Ensure consistently high standards of record/case note recording, maintaining electronic and integrated records and case notes in accordance with the standards policy and relaying verbal information where appropriate.
  • Maintain confidentiality and security of information in all matters relating to patients.
  • Attend training courses and participate in personal development training as identified by the Team Leader, maintaining responsibility for ensuring mandatory training is up to date.
Eligibility and Requirements

Applications from candidates that require current Skilled Worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service can present a valid DBS certificate instead of requiring a new check.
