A community support organization in Hull is seeking a Community Practitioner to deliver intensive care and support to individuals with complex needs in their homes. Responsibilities include conducting needs assessments, engaging with customers to promote independent living, and ensuring a high standard of service. The ideal candidate will be caring, knowledgeable about welfare and mental health issues, and possess strong interpersonal skills. A full-time position with comprehensive training and benefits is offered.
We are currently looking to recruit a Community Practitioner on our Self-Enablement Service.
The Service is aimed to respond to people aged 16 plus that present with complex care and support needs. It involves delivery of Intensive Complex Care and Support to individuals in their own homes or accommodation sourced by Humbercare, many stepping down from long term hospital or residential placements, those leaving the care system or secure or prison stays; working to develop people’s capacity to live independently.
Please note that this is a 40-hour position, and will include an element of on-call (which may cover evenings and weekends) in addition to the core hours.
