Job summary

A community support organization in Hull is seeking a Community Practitioner to deliver intensive care and support to individuals with complex needs in their homes. Job description
Community Practitioner

We are currently looking to recruit a Community Practitioner on our Self-Enablement Service.

The Service is aimed to respond to people aged 16 plus that present with complex care and support needs. It involves delivery of Intensive Complex Care and Support to individuals in their own homes or accommodation sourced by Humbercare, many stepping down from long term hospital or residential placements, those leaving the care system or secure or prison stays; working to develop people’s capacity to live independently.

The main responsibilities include:
  • To carry out needs and risk assessments, conduct support reviews and provide care and support to all customers accepted onto the Self-Enablement Service
  • To engage with customers on an agreed basis to support individuals to achieve and maintain independent living
  • To provide support in accordance with current best practice, according to policy and procedures, agreed standards, legislative requirements, relevant regulations under the direction of the Manager
  • To provide a high-quality service; ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and point of contact for customers. Engaging with customers to design services that work for them and utilising resources within the local area to make a real, quality difference to peoples' lives
The successful candidate must:
  • Be a supportive and caring individual
  • Have work experience of delivery of Personal Care and Support
  • Knowledge of welfare rights, tenancy rights and responsibilities
  • Have the ability to treat people with dignity and respect
  • Experience of Needs and Risk assessing, Support Planning, crisis input and engaging with customers in order to deliver positive outcomes
  • Have knowledge of Mental Health Issues and diagnosis, Substance Misuse issues and the needs of customers that have gone through the Criminal Justice System
  • The ability to communicate at all levels and to develop and sustain a working relationship with customers and colleagues
  • The ability to work in a team as well as work on own initiative
  • The ability to undertake various shift patterns on a planned rota including days and nights
  • Have good interpersonal skills
  • Have compassion and empathy
  • Be flexible and committed

Please note that this is a 40-hour position, and will include an element of on-call (which may cover evenings and weekends) in addition to the core hours.
