Overview

We are currently recruiting a Support Practitioner on a full time basis for our housing related support framework service. The service is registered with the Care Quality Commission to deliver Personal Care.

Responsibilities
  • Maintaining and managing independent accommodation
  • Setting up and maintaining essential utilities
  • Accessing specialist advice or agencies
  • Maximising benefit entitlements
  • Money and debt management
  • Liaising with other professionals
  • Accessing training, voluntary work, education and employment
  • Developing independent living skills
  • Access to client involvement groups
  • Encouraging positive neighbour relationships and involvement in the local community
  • Appropriate use of leisure time
  • Personal Care
Qualifications
  • An understanding of Person Centred approaches to care and support
  • An understanding of Risk Management
  • The ability to engage with a complex and challenging Service User group
  • The ability to communicate at all levels
  • Experience of supporting vulnerable adults within their own homes
  • Work positively with new demands and changes
  • The ability to treat people with dignity and respect

  • The ability to work within a team and on your own initiative
Salary

Salary - £26,208.00 per annum
