An opportunity has arisen for a Support Planner to work within Caring For Thurrock. The role is to work alongside team members who are responsible for managing the staff and service users within the service. Caring for Thurrock is the internal care department of Thurrock Council Adult Social Care and is responsible for provision of Care for Thurrock residents. The team is a Multi Disciplinary Team working alongside colleagues within the Health Service in providing long term and reablement care to service users within Thurrock. The team focuses on responsive social care support that creates opportunity for vulnerable people to come together with their community, while providing support that is committed to safeguarding the most vulnerable.
As part of the Caring For Thurrock team you will be working in a way that is responsive and proportionate, working with people to achieve their outcomes using their strengths and their community creativity, and linking people with their communities focusing on 'what matters rather than what's the matter'. You will need to be an excellent communicator and able to work alongside colleagues in other departments, health, community partners and agencies in order to meet the care and support needs, and outcomes for our residents.
