We are looking for a caring, dynamic and highly motivated person to join the team as a Support Coordinator in our Fletcher Court & South Manchester Community Outreach services in Wythenshawe. This is an exciting opportunity to be part of our well-established team, which supports individuals with mental health needs.

You must have skills in positive communication and engagement and will be able to demonstrate unconditional regard for the people we support. You will also have skills in the assessment of needs and proactive risk management, so that support planning for individuals demonstrates an active and fulfilling life with an emphasis on a ‘can do’ approach.

Responsibilities
  • Developing the independent living skills and community inclusion of service users
  • Identifying and accessing activities with service user to promote valued and meaningful lifestyles
  • Providing respectful personal care, emotional and practical support to service users
  • Collaborating with service users, families, and the multi-disciplinary team to maximise service user outcomes
  • Implementing Creative Support principles, policies, procedures and guidelines and ensuring that the service fulfils Manchester City Council (MCC) requirements
  • Supervising and supporting staff to best meet service user needs and preferences
  • Supporting service users with behaviour that challenges services and implement positive behaviour support

Experience is essential for this role, as is access to a car. You will be expected to work flexibly to meet the needs of the service, including evenings and weekends.
