Overview

The successful applicant will support patients undergoing regular haemodialysis treatment which will involve a multitude of skills including patient education and application technology. Initiate and administer haemodialysis treatment via subclavian catheters, arterio-venous grafts and fistulas, administer medications, fluid therapy, blood products. Work in a fast paced environment with a multidisciplinary team to provide haemodialysis care services in the clinical environment. Perform patient care activities such as taking vital signs and initiating, performing and stopping dialysis treatments. Manage, direct and evaluate the nursing care for a defined caseload of patients.

Responsibilities
  • Develop skills to provide senior nurse cover for the ward in the absence of the Ward Sister or Charge Nurse and Team Leaders.
  • Contribute towards the continuing development of professional nursing practice.
  • Maintain and develop high standards of nursing care, reflecting the beliefs about professional nursing practice and patient care outlined in the ward philosophy.
  • Develop and maintain specialist knowledge of nursing practice.
  • Involve and encourage the patients family and friends to be actively involved in the caring process.
  • Communicate effectively information to patient/relatives/carers/ and all members of the multidisciplinary team.
  • Work towards acting as a mentor and role model to others, reflecting the beliefs about professional nursing practice and patient care outlined in the ward philosophy.
  • To conduct oneself in a professional manner and act as a role model to junior staff at all times.
  • Be proactive in the timely discharge planning of patients.
  • Clinical Practice: Demonstrate and promote a patient centred style of clinical practice.
  • Be responsible for the assessment, planning, implementation and evaluation of patient care from admission to discharge.
  • Respond calmly to emergency situations, maintaining patient safety and seeking immediate support.
  • With supervision and assistance, take on increased responsibility within the unit structure, acting as a named nurse recognizing own limitations and seeking help appropriately.
  • Promote shared governance and set and maintain standards of practice, conduct and decision making (as negotiated with Team Leader/Ward Sister).
  • Accurately document on EPR and report information regarding the care given to patients to the nursing team and multidisciplinary team, communicating changes as they occur.
  • Identify and discuss nursing/patient needs with team/ward staff, patients, carers and the multidisciplinary team. Plan own workload.
  • Communication and Professional Practice: Adhere to the NMC Code of Conduct and practice within the NMC Scope of Professional Practice, maintaining patient confidentiality; adhere to data protection.
  • Demonstrate a problem solving approach to planning, implementation and evaluation of individualised plans of care based on a holistic model of care.
  • Demonstrate accountability, responsibility and duty of care and support for junior staff on practice implications.
  • Effectively communicate with patients, visitors and colleagues to promote comfort and wellbeing, considering barriers to communication and language needs.
  • Resolve conflict and maintain patient dignity and confidentiality.
  • Teamwork, Leadership and Management: Develop skills to act as Ward Team Co-ordinator in rotation, contribute to ward projects and audits, and supervise junior nurses as appropriate.
  • Ensure equipment, disposables and staffing resources are used appropriately when prescribing and implementing care and treatments.
  • Report untoward incidents and aid investigation as required; comply with health and safety regulations and ward protocols.
  • Maintain and contribute to standards of practice in conjunction with team leaders/ward manager; participate in personal development planning and annual appraisal.
  • Support ward education programmes and competency frameworks; complete local Trust competency frameworks; develop skills to supervise and mentor junior staff; participate in mandatory study days and research-based practice.
  • Develop knowledge and competence in the speciality; act as mentor to students and colleagues in orientation programmes; contribute to practice improvement and clinical governance.
  • Improve practice through evidence-based nursing care and participate in audit and research; keep up to date with professional issues affecting nursing practice.
  • Openly question and challenge nursing practice to enhance patient care quality; maintain a record of personal professional development.
  • Negotiate an area of clinical interest with Team Leader and develop resources for staff learning.
