Exciting opportunity for a Social Worker to join the WISH Team (Wokingham Information and Support Hub) in Wokingham, UK. This full-time locum position offers an attractive hourly rate of £35.89 and is a 3-month contract. In this role, you will be a key player in a vibrant team focused on empowering individuals with learning disabilities, physical disabilities, long-term conditions, young carers, those transitioning into adulthood, and older people. What you will do:

  • Provide direct support to adults with learning and physical disabilities, long-term conditions, and those transitioning into adulthood or ageing.
  • Evaluate the needs of customers and create tailored plans to enhance their independence and wellbeing.
  • Conduct regular community visits and ensure the delivery of high-quality care and support services.
  • Collaborate with multidisciplinary teams to address complex issues such as homelessness, dementia, substance misuse, and hoarding.
  • Perform carers assessments and offer guidance and support for those in caring roles.
  • Be office-based at least once a week and use your own vehicle to visit clients on other days.

Candidate requirements:

  • Qualified social worker with over three years of experience in adult social care.
  • Sound understanding and experience in dementia care, addressing drug and alcohol-related issues, and working with hoarders.
  • Experience in tackling homelessness issues and conducting S42 enquiries.
  • Strong background in community work and multi-disciplinary collaboration.
  • Must hold a valid driving licence and have access to a car.
