What you will do

• Act as the lead for up to six young people, delivering proactive and flexible interventions using the Breaking Cycles model.
• Carry out statutory social work duties, working closely with each young person to identify their motivations and support their journey towards success.
• Engage with families and professional networks to tackle any barriers or underlying trauma affecting the young people.
• Provide support in various settings, including home, school, and community environments, even outside traditional office hours and at weekends.
• Participate actively in weekly practice discussions and contribute to data evaluation, fostering a culture of continuous learning and improvement.
