Job description

Works for the Central Virginia VA Health Care System in Richmond, Virginia, as a direct report to the Social Work Service, and is assigned to the Caregiver Support Program under the Program of Comprehensive Assistance for Family Caregivers.

Responsibilities

The Caregiver Support Program Coordinator for the PCAFC is a Social Work Program Coordinator whose primary responsibilities are to provide clinical evidence‑based services and interventions; program development; caregiver, Veteran and staff education on caregiver issues; community outreach; resource development; continuous quality‑improvement activities and evaluation/consultation related to the PCAFC. The Coordinator will function in support of caregivers, Veterans and family members throughout the Medical Center and in the community and will support the development and implementation of General Caregiver services. This includes working with the facility CSP Program Manager to provide program support and oversight of the PCAFC under the umbrella of Caregiver Support Program services. Tasks may include tracking consults and workload, assisting with new staff orientation, conducting frequent needs assessments to implement education, wellness, and support groups, and building partnerships with local, state, federal agencies, and non‑governmental organizations.

The Coordinator is administratively responsible for the clinical programming and daily operations of the PCAFC and may serve as back‑up to the facility CSP Program Manager. The Coordinator assists with the development of policies and procedures for program operation, prepares reports and statistics for facility, VISN, and national use, and may be responsible for the program's budget and various control points assigned to the program within the assigned areas. The Coordinator manages and controls the use of data related to these programs regarding day‑to‑day functioning and program evaluation, plans, organizes and directs administrative, operational, caregiver, Veteran and personnel activities for the program. Performs other directly related duties as assigned.

Work Schedule

Monday‑Friday, 8am - 4:30pm.

Telework / Virtual

Ad‑Hoc telework only to support continuing operations on a non‑routine basis, as determined by the agency policy. This is not a virtual position.
