Overview

Social Worker – Learning Disabilities Duty Team, Hackney. Full-time position for 3 months at £32 hourly.

This role involves joining Adult Services to provide crucial social work services to adults with learning disabilities. You will manage a varied caseload, conduct comprehensive assessments, and craft tailored care plans based on the principles of the Care Act, ensuring timely responses to the needs of adults, carers, and their families.

Responsibilities
  • Provide high-quality social work services to adults with learning disabilities
  • Participate in assessments, care planning, and safeguarding
  • Conduct robust assessments with service users, carers, and multi-disciplinary partners
  • Deliver services in line with the wellbeing principles of the Care Act
  • Promote personalised support, facilitating service user choice and control
  • Manage a caseload effectively, clearly identifying outcomes to be achieved
  • Help service users identify long-term needs and aspirations
  • Develop outcomes-focused support plans based on asset-based goals
