  • Contract: Temporary / Agency
About Us:

Cheshire West and Chester is seeking an experienced Adult Social Worker to join our Continuing Health Care (CHC) team. This is a vital role supporting adults with complex needs and ensuring high-quality, person-centred decision-making in line with national CHC frameworks.

Within the framework of legislation and agreed Council policies and procedures, you will work with adults and their families to address personal challenges, support them in adjusting to or improving their social environment, and ultimately enhance their overall quality of life.

Key Responsibilities:
  • Assess need for social work intervention, including identifying risk and safeguarding concerns, determining appropriate methods of intervention, and advising on suitable social care or external resources.
  • Liaise and negotiate with health professionals, statutory bodies, and voluntary agencies to ensure coordinated and effective service delivery.
  • Manage and monitor a defined caseload, working within team priorities, Council policies, and statutory responsibilities.
  • Maintain accurate case records and complete all required administrative and reporting procedures.
  • Participate in supervision, team meetings, and training, making use of development opportunities to enhance practice.
  • Contribute to service improvement, sharing knowledge, skills, and best practice with colleagues, partner agencies, and relevant professional groups.
  • Supervise students, where appropriate, supporting learning and professional development.
