Overview

Caerphilly County Borough Council is seeking an experienced and dedicated Social Worker to join our 16 Plus Team. This is a fantastic opportunity to work with young people transitioning into adulthood, ensuring they receive the right support to achieve independence and stability.

You will play a key role in supporting care-experienced young people, providing high-quality assessment, care planning, and intervention. The role involves working closely with multi-agency partners, advocating for young people, and ensuring they have access to education, training, housing, and emotional well-being support.

Responsibilities
  • Provide high-quality assessment and care management for young people requiring ongoing intervention.
  • Review care plans within statutory timescales, ensuring they align with the young person\'s needs and aspirations.
  • Work in partnership with education, housing, health, and support services to promote positive outcomes.
  • Offer guidance and practical support to young people, empowering them to achieve independence.
  • Represent the service at court hearings, panel meetings, and multi-agency forums.
  • Ensure effective multi-disciplinary collaboration with partner agencies.
  • Contribute to service development initiatives to enhance outcomes for young people.
  • Manage your caseload with supervision and support, ensuring accountability in your practice.
Requirements
  • Qualifications: Degree in Social Work and registration with Social Care Wales.
  • Experience: Experience working with care-experienced young people, safeguarding, and care planning.
  • Skills: Strong assessment, advocacy, and communication skills.
  • Knowledge: A sound understanding of legislation, policies, and best practices related to looked-after children and care leavers.
  • Other Requirements: Ability to work effectively as part of a multi-agency team and support young people through complex transitions.
