What you will do

• Conduct timely and efficient assessments and support planning for service users recently discharged through the Discharge to Assess pathway.
• Be adept at carrying out duty and welfare checks with Home First to facilitate seamless discharges from D2A beds.
• Complete Care Act 2014 assessments and other necessary evaluations such as Mental Capacity Act Assessments and Continuing Healthcare Checklists.
• Actively participate in CHC Decision Support Tool meetings, contributing your expertise to ensure comprehensive care planning.
• Travel throughout Herefordshire as needed to conduct in-person visits, armed with a strong knowledge of both the Mosaic record-keeping system and Microsoft Office applications.
