What you will do:
  • Conduct complex assessments to identify appropriate services for individuals, ensuring all is within budget guidelines.
  • Supervise team members and participate in departmental training and development activities.
  • Evaluate and confirm eligibility for client assessments, collaborating with clients and their carers to develop comprehensive care plans.
  • Engage with markets to secure cost‑effective resources for client care, ensuring plans meet both needs and budgetary constraints.
  • Regularly monitor and reassess clients’ needs, adapting care plans in consultation with clients and carers when necessary.
  • Provide ongoing advice and support to clients and note any service delivery issues to the Team Manager.
  • Undertake duties under the Mental Health Act where required.
  • Record and maintain all necessary documentation in line with departmental procedures.
  • Participate in the Team Duty System and contribute to quality standards and performance measures.
Requirements:
  • Robust knowledge of Care Management, Mental Health Act, and Code of Practice.
  • Proven experience in Social Care Services, preferably within Mental Health and Community Mental Health Teams.
  • Relevant qualification, such as CQSW, CSS, or a diploma in Social Work. ASW accreditation is desirable.
  • Strong understanding of anti‑discriminatory and anti‑racist practice.
