We’re seeking an experienced Adult Social Worker to join the Older Persons and Physical Disability Service in Southwark. This full-time locum position offers a competitive £33 hourly rate and the opportunity to manage a varied caseload within a progressive and supportive team.

What you will do

Manage a diverse and complex caseload of older persons and adults with physical disabilities.

Carry out Care Act, Carers’, Mental Capacity, and CHC assessments.

Present and gain approval for care packages, arrange care home placements, and manage safeguarding concerns.

Build and maintain strong partnerships with GPs, housing services, occupational therapists, and other stakeholders.

Use case management systems to document and track case progress effectively.
