Location: Adults Community Team, West Lothian, UK. Pay Rate: £37.00 per hour. Contract Type: Locum (Ongoing). Working Pattern: Full-time.

You will be part of a collaborative and supportive Adults Community Team, working to empower adults and support them in achieving positive outcomes in line with the Care Act 2014.

Job Purpose

To deliver high-quality, person-centred social work services to adults with care and support needs, as well as their families and carers. You will work as part of a multi-disciplinary Adults Community Team, supporting independence, protecting vulnerable adults, and helping people achieve their personal goals in line with statutory duties and local policies.

Key Responsibilities

Conduct strengths-based assessments in accordance with the Care Act 2014, including risk and Mental Capacity Act assessments.

Co-produce personalised care and support plans that promote wellbeing, independence, and meet eligible needs.

Undertake safeguarding enquiries under Section 42 of the Care Act, ensuring robust responses to protect adults at risk.

Work in partnership with professionals from health, housing, and the voluntary sector to provide integrated, person-centred care.

Manage a varied caseload of complex cases with autonomy and accountability.

Act as an advocate for service users, ensuring their voice and preferences are heard and respected.

Maintain accurate and up-to-date case records using the electronic case management system.

Participate in supervision, team meetings, and continuous professional development in line with Social Work England requirements.

Contribute to service development and improvement initiatives.

Person Specification

Qualified Social Worker with current registration with Scottish Social Services Council (SSSC).

Demonstrable experience in adult social care, ideally within a community setting.

Solid understanding of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and the Human Rights Act 1998.

Ability to carry out assessments and manage risk effectively in complex scenarios.

Excellent communication and interpersonal skills, with the ability to build trusting relationships.

Experience of working collaboratively with service users, families, carers, and other professionals.

Strong organisational skills and the ability to work independently.

Proficient in IT and experienced in using electronic case recording systems.
