Overview

We are working closely alongside a Local Authority in The Royal Borough ofGreenwich to assist with the appointment of a Social care Assessor to work in a residential care setting.

Summary:

The Social Care Assessor will work directly with adults, older people, and/or their carers to assess social care needs, identify risks, and support the creation and review of care and support plans. The role involves working in partnership with individuals, families, carers, and professionals to promote independence, safeguard wellbeing, and ensure personalised, high-quality care in line with legislation, policy, and best practice.

Rate of Pay

£18.80 - £23.67 per hour

Responsibilities
  • Conduct assessments and reviews of care and support plans for adults with social care needs, enabling independence, wellbeing, and active community participation.
  • Work creatively and collaboratively with service users, families, carers, and other professionals, using person-centred and evidence-based approaches.
  • Ensure safeguarding of adults and children at risk by gathering and sharing information appropriately, escalating concerns, and supporting safeguarding enquiries under supervision.
  • Take responsibility for personal professional development, attending training and undertaking annual performance reviews.
  • Contribute to policy development, practice improvement, and the training and guidance of colleagues and external partners.
Qualifications
  • A-levels (or equivalent) in a relevant field
  • NVQ Level 4 / QCF Level 4 Diploma in Health & Social Care, or an associated degree
Essentials
