The Role

Job Title : Senior Support Worker

Hours : 37.5 hours per week

Shift Pattern : Monday to Friday, 9:00 am to 5:00 pm

We're looking for a Senior Support Worker to join our Rotherham Hospital Discharge service. The service offers comprehensive housing‑related support to adults on inpatient wards at Swallownest Court. This initiative is driven by our mission to facilitate successful reintegration into the community.

We're searching for an energetic, compassionate, and exceptional Senior Support Worker who is driven by the desire to make significant and lasting differences in people's lives. This is an opportunity to be part of something meaningful, transformative, that can truly change lives.

In this pivotal role, you'll be at the heart of the action, coordinating the daily operation of the service. You'll also serve as a Keyworker for a dedicated caseload, providing much‑needed support in both hospital and community settings, including within people's homes.
