We are seeking individuals who can bring a wealth of knowledge and experience in social work. We are looking for a passionate, motivated, and experienced Grade 10 Senior Social Worker to join our Hospital Social Work Team. Your role will involve working with Adults aged over 18 to facilitate a safe transfer of care from hospital. The team also completes Care Act Assessments once back within the community to assess long-term care needs. You must enjoy being part of a fast-paced environment and working alongside our multi-disciplinary system partners.
