Overview

We are currently looking for a Senior Physiotherapist to join our Intermediate Care Team in the South, based in Cambridgeshire. If you are dynamic, flexible and enjoy variety in your work and wish to be part of a team making a difference in our community, you are the right person for us.

The vision of the service is to provide outstanding, patient-focused, community based health care and therapy, optimising peoples potential to achieve their goals by supporting earlier discharge from hospital using the home first model.

Responsibilities
  • To provide comprehensive, holistic therapy assessment and rehabilitation to patients within their home environment, maximising independence within a 4-week pathway.
  • To hold clinical responsibility for patients within the service and Therapy Practitioners within their team. To prioritise workload including that of the Therapy Assistants within the team.
  • To work proactively, in collaboration with the wider multidisciplinary team to ensure patient flow and safety.
  • To assist in the development of the Integrated Care Workers and Therapy Practitioners to support the clinical delivery of the service.
  • To be professionally and legally accountable for all aspects of your own and delegated work.
  • To work within codes of practice and professional guidelines.
  • To undertake holistic assessment (including those with complex presentations and multi-pathologies) making use of specialist clinical reasoning skills.
  • To use evidence-based practice to ensure clinical care is effective and appropriate.
  • To promote independence and wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive the appropriate care in the most appropriate setting.
  • Organise and manage own time, delegating work appropriately.
  • Conduct risk assessments and health and safety assessments and report any changes/newly identified risks.
  • Demonstrate the importance of gaining patient and carer consent in all interventions.
  • Effectively communicate verbally with patients, carers and colleagues using tact and persuasive skills, including with patients who have communication difficulties.
  • Actively involved in the development of the service/ organisational aims and objectives of the Teams and the new model of care.
  • Work effectively within the team structure and liaise with the MDT and other agencies by attending MDT meetings, case reviews.
  • Ensure that accurate and timely written records are kept which comply with the Trust policy and HCPC guidance, reporting on any issues as appropriate.
  • Provide specialist advice to all members of the multi-disciplinary team, service users and others on changes involving current care plans, patients progress and other relevant matters that pertain to the care of the patient.
  • Support day-to-day planning and delivery of high‑quality care.
Qualifications & Requirements
  • Licensed physiotherapist with appropriate professional registration and ongoing professional development.
  • Experience working as a physiotherapist within community/ home-based settings is desirable.
  • Ability to travel independently around the county and meet strict time deadlines; full UK driving licence and access to a vehicle may be required.
  • Excellent communication and interpersonal skills; ability to work effectively within a multidisciplinary team.
