A regional healthcare provider is seeking a Band 6 Senior Community Nurse to lead in the Hospital at Home service across Worcestershire. This full-time role includes independent care management, supporting acutely unwell patients within the community. We have an exciting opportunity for a Band 6 Senior Community Nurse who has a passion for admission prevention to join a core Team of ACPs to deliver a new and developing countywide Hospital at Home (H@H) service in Worcestershire. This is a full time post, but we would consider applicants looking for part time. The post is based in the Single Point of Access (SPA) Worcester and will be the clinical coordinator for the H@H service. You will be managing referrals in and out of the service as well as managing the daily capacity and caseloads. There will be the opportunity to do clinical visits if and when required to support the delivery of the service and patient care as required. This is an exciting opportunity to be part of the delivery of a new service and to play a significant part in shaping and developing this service. There will be the opportunity to partake in audit and service development as well as the clinical duties of the post. There will also be the opportunity to work collaboratively with stakeholders in particular with Acute Trust colleagues. The service will be countywide across Worcestershire so the ability travel across the county will be expected. The coordinator role will be expected to cover 0900-1700 seven days a week including Bank Holidays so the post holder will be expected to work shift patterns to enable cover of the operational hours.
