Devon Partnership NHS Trust is seeking an experienced and forward-thinking Senior Clinical Pharmacist to join our expanding Medicines Optimisation service.
The post holder will provide a specialist clinical pharmacy service to mental health community rehabilitation teams across the Trust, supporting safe, effective and person-centred use of medicines for people with complex mental health needs.
You will work closely with people who use our services, their relatives and carers, offering expert advice, optimising medication regimes and promoting shared decision-making. You will collaborate with a wide range of colleagues, including the DPT Medicines Optimisation team, multidisciplinary community and inpatient teams, Consultant Psychiatrists, prescribers, nurses, OTs, psychologists and social workers. Strong links will also be maintained with PCN GPs, primary care pharmacy teams, and specialist mental health and learning disability pharmacists across regional networks.
