Job description

Responsibilities

• Assist patients with daily living activities, including personal hygiene, dressing, and mobility.
• Medication trained.
• Support patients in following their individual care plans, ensuring all needs are met effectively.
• Communicate clearly with patients and their families to provide updates on care and address any concerns.
• Maintain accurate records of patient progress and report any changes in condition to healthcare professionals.
• Collaborate with other healthcare team members to ensure a holistic approach to patient care.
• Utilise IT systems for documentation and communication purposes, ensuring confidentiality is maintained at all times.
