Senior Care Co-ordinator – Home Care Services
Permanent, Full Time
Responsibilities:
- Senior Care Co-ordinator responsible for the day to day administration of the Home Care Service – this is a new role and service for Dorking.
- Assist the Registered Manager in commissioning the new service, develop the service user base, staffing model and all systems and processes prior to the business going ‘live’.
Key areas of responsibility:
- Initial assessments of service users to identify needs, goals and resources.
- Develop individual care plans.
- Organise clear schedule for service users and staff.
- Act as liaison between service users and the Society.
- Monitor care progress and adjust plans accordingly.
- Maintain accurate and timely documentation.
- Ensure compliance with relevant policies, standards and regulations.
- Participate in an on-call rota to ensure 24/7 support.
Relationship management:
- Work effectively with other colleagues to ensure the service user’s best interests are identified and plans established to meet them.
- Act at all times as an ambassador for the Society.
Operational management:
- Actively build a strong service user base to facilitate the continuation of the business on a sound financial footing.
- Ensure full compliance within the service and ensure all legislative reporting requirements are met.
- Be responsible for overseeing the development of Home Care staff.
- Develop and maintain communication practices.
Leadership:
- Day to day supervision of staff and ensure all staff receive regular Support and Supervision.
- Identify staff training needs.
- Oversee shift rota system.
- Ensure appropriately skilled staff are recruited and trained and staff meet CQC requirements.
Care of Service Users:
- Create and manage schedules for care visits ensuring service users receive the necessary care according to their care plans.
- Be a central point of contact for service users, staff and healthcare professionals.
- Monitor quality of care delivered, ensure it meets required standards, address improvement areas and carry out spot checks on staff.
- Ensure staff are aware of the Person Centre Care concept and adopt appropriate working practices to facilitate it.
- Stand in to cover in emergency situations when required.
Admin Duties:
- Develop and maintain suitable record keeping system.
- Produce data, statistics and reports as required.
Other:
- Promote, maintain and evaluate the service’s delivery.
- Undertake training as required.
- Actively promote the concept of equality, diversity and human rights in all aspects of the service’s work.
- Keep up to date with all appropriate legislation, thinking and theory in relation to professional conduct and practice and ensure that staff are familiar with these.

Requirements:
- NVQ3 in Health and Social Care or equivalent is essential.
- Level 5 Diploma and further management training is desirable.
- Minimum of 2 years working in a similar role in the Home Care sector.
- Able to work at short notice when required.
- Able to take on on-call duties when required.
- Live in a 10 mile radius of Dorking.
- Care experience.
- UK driving licence and own car essential.
