This role offers the chance to work with a varied caseload of patients receiving acute medical care in their own homes, allowing for a truly personalised approach to treatment. The successful candidate will be based at the Woodend Hospital site in Aberdeen and will play a key role in ensuring patients receive the highest standard of assessment, care, and treatment. The post holder will work both independently and as part of a multidisciplinary team, collaborating with Consultants, GPs, Advanced Nurse Practitioners, Nurses, Physiotherapists, Pharmacy staff, Occupational Therapists, and Health Care Support Workers.
Work independently and as part of a multidisciplinary team to deliver high-quality patient care in patients\' own homes. Provide comprehensive assessment, care, and treatment to patients receiving acute medical care in the community. Contribute to care planning and ensure patient-centred approaches in the home setting. Collaborate with a wide range of health professionals to support patient outcomes.
We are seeking motivated team members with a commitment to delivering high-quality patient care. Preference will be given to applicants with current hospital or community experience. Applicants must meet the requirements of the person specification to be considered for these posts. Part-time applicants are welcomed and are encouraged to identify their preferred working hours within their application. Hospital at Home staff nurse working hours are from 0800 to 2015.
Essential: Full UK driving licence and access to a car for work purposes; be prepared to insure the car for business purposes. Ability and willingness to travel throughout Aberdeen City and Aberdeenshire. Please consider the implications of the LEZ zone in Aberdeen city if your car is not compliant.
