Qualifications

• Minimum of 12 months experience working in general practice/primary care.
• Registered Clinical Pharmacist with GPHC registration.
• Desirable to be on CPPE Primary Care pathway or willing to partake.

Responsibilities

• Support and work alongside a team of pharmacists and GP's.
• Conduct polypharmacy medication reviews.
• Educate patients on medication and disease prevention.

Skills

Excellent communication and interpersonal skills

Experience with EMIS

Experience with System 1

Ability to work within professional boundaries

Education

Degree/qualification as a registered Clinical Pharmacist with GPHC
