A healthcare organization in the UK is seeking a Band 4 Rehabilitation Assistant. You will support neuro rehabilitation for patients in their homes and the community. Responsibilities include assessing needs, managing a caseload, and working with multidisciplinary teams. A full UK driving licence is required for independent travel across the work locality. The ideal candidate should be dynamic and adaptable with a focus on patient-centered care.
You will have opportunities to work closely with multidisciplinary teams in the community, primary care, the acute neuro rehab team, the stroke early supported discharge team and other external agencies. The post is within a multidisciplinary team and you will work alongside Neuro Physiotherapists, Occupational Therapists, Psychologists, Speech and Language Therapists and Rehab Assistants. The post is based at Hinchingbrooke Hospital, Huntingdon with potential to cover across the South of the county and county-wide duties as required. You will work in an agile manner using virtual consultation to manage your caseload, and you will have access to a smart phone and laptop. You will be required to travel independently around the county to meet strict deadlines.
We are seeking a dynamic Band 4 Rehabilitation Assistant to support neuro rehab as in-reach to inpatient rehab beds and with patients in their own homes. You will support patients with physical, cognitive and psychosocial challenges as part of a MDT to achieve their potential and improve health, care and outcomes.
